Advice in scheduling:

We advise to choose the amount of time needed for the extent of documents, history and questions you may have. New client consultations typically take 60 minutes. If you believe you will need further support, ensure you are scheduled for future consultation(s) as Dr. Story’s schedule can fill up quickly. If you believe you need only a quick consultation, you can set up a 30 minute new client appointment. Be advised this will need to be a concise visit. If you are needing more time, please consider this when scheduling. If more questions arise, Dr. Story may not be able to extend your visit. If she does have time, you will be asked to pay for the difference at the end of your visit. Time is billed in 15 minute increments. You are welcome to schedule additional time after your initial visit.

You will be sent an email link to set up your Charm client portal. There you can upload any documents you would like to review with Dr. Story during your consultation. You are responsible for filling out and uploading your intake form on the Forms page. We highly recommend uploading this and any documentation you would like to share with Dr. Story prior to your scheduled appointment to conserve time for discussion. Lastly, please read the cancellation policy on this page to be aware of how to cancel or reschedule if needed to avoid charges. Please note the time zone shown when scheduling.

Any time you need to set up a follow up consultation, you can come back to this page to do so. Please do not try to schedule through the Charm portal at this time as it is not synced for client consultations.

 

Fees

Please note: Fees have been recently updated as of December 2024.

Distance educational consultations cannot be billed to Insurance or HSA accounts, as these are not medical services. While we cannot provide a superbill, you can request an receipt. All consultations are paid for upon scheduling. 

Cancellation Policy

If you need to cancel or reschedule your appointment, please contact us at drstory@drmistystory.com. Please note, we have a 48 hour cancellation policy. Changes or cancellations less than 48 business hours (M-F) before your visit will incur a $50 late cancel/change fee. The full visit fee will be charged for clients who fail to attend their appointment, or fail to cancel prior to the appointment. We do understand there can be extenuating circumstances resulting in late cancellations or missed appointments; please contact us if this occurs.
